If you are seeking disability, please let me know prior to your appointment. Your initial evaluation does not cover information needed for a disability evaluation If you are requesting assistance in completion of a disability claim, please make note of the following:
1. I do not “give” disability. I can only report symptoms and response to treatment the company that handles your disability insurance. 2. If you are disabled for work, my goal is to help you return to work. I will help with issues that are preventing you from being able to work. 3. In conjunction with a therapist I will sign-off on disability reports if deemed appropriate by thet herapist and myself. 4. I do not back date disability. 5. More frequent appointments will be necessary to aggressively treat your developmentalstatus. The goal is to effectively and rapidly improve your condition to the point that you will be able to return to work
6. If regularly scheduled appointments are not kept, I will notify your disability carrier and terminate your care. |
