CODE OF ETHICAL BEHAVIOR • I will always provide my services with respect for human dignity and the uniqueness of the consumer, regardless of the individual’s social or economic status, personal attributes or the nature or degree of the disability. I will act to safeguard the consumer’s right to privacy and dignity. To protect the consumer and the public from the incompetent, unethical or illegal practice of any person. I will not allow personal interests to conflict with my duties.
REGARDING INSURANCE: I do not accept insurance at this time. I may be listed as a covered provider by your insurance company in their booklets and websites. Unfortunately, simply because I am listed does not mean that your plan will pay for or authorize your visits. I see patients on a cash basis. If you have insurance, you should contact your insurance company to inquire about out of network coverage and to determine if you can submit my bills directly to get some financial reimbursement for out of network services provided. Please note, I do not file insurance. I will provide you with a signed stamped receipt for service upon checkout that you will be able to submit to you insurance provider for reimbursement should you choose to do so. If you have insurance that has “out of network” benefits, they will typically reimburse 30% – 80% of my fees. Even though I do not accept insurance, there may be times when your provider will need my office to call them for authorization and approval of certain medications. To provide this service I will need the information from your insurance card. Please provide a copy of your driver's license and up-to-date insurance card at you first visit. If your insurance provider changes, please provide us with the new card as soon as it is available. |
